What to bring
Please bring the following to your appointment:
- Photo ID
- Insurance card(s)
- A complete list of current medications
- Any relevant medical history
- Referral slip (if provided)
- Previous X-rays or imaging, if available
Forms & paperwork
To save time, we strongly encourage completing your health history and registration forms before your appointment.
Incomplete forms may delay your appointment or treatment.
During your visit
Your appointment will typically include:
- Review of your medical and dental history
- Clinical examination
- Necessary imaging (panoramic and/or 3D CBCT if indicated)
- Discussion of findings and treatment options
Most consultation visits take about 30-60 minutes.
At the conclusion of your appointment
Before you leave, we will:
- Create a personalized treatment plan
- Discuss risks, benefits, and alternatives
- Answer your questions so you can make an informed decision
- Review and sign consent forms if proceeding
- Provide a detailed financial estimate
- Discuss insurance coverage and payment options
