Prior to your appointment:
Please complete your registration and health history forms from home prior to your first appointment, or arrive at least 15 minutes early to complete them in office.
At your first appointment:
Your first appointment will be a consultation, where we will review your dental and medical history, your treatment and sedation options, and address any questions or concerns you may have. If we need additional x-rays, we will take them at this time. We will then schedule the appointment for your oral surgery procedure.
- All patients under the age of 18 will need to be accompanied by a parent or legal guardian.
- A preoperative consultation and examination is mandatory for patients undergoing IV anesthesia for surgery.
- Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medications (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
- Please bring with you:
- Insurance Card
- Referral Slip and x-rays
- List of Medications
